does anyone know where i can find a php/mysql database testing script.

I am teaching a group of students how to use the internet and i want to know if there is an open source free php script which i can use for my students to i can make up questions and they will take the test and score their results automatically. i want to be able for each page to have its own question until the end when they finish they'll be able to score the results. a questionaire script like this would be very helpful for my students to learn and practice on an exam.
